Care Resource Management; “We Connect the Dots”
The Care Resource Management staff help patients navigate a complex healthcare system and coordinate their care needs.

Case Management
• Assists patient in planning for their recovery upon discharge.
• Identifies discharge needs and arranges for equipment and/or assistance if indicated to assure continuing care needs are met.
• Works closely with the physicians and the healthcare team to coordinate patient care.

Social Services
• Assists patients in coping with their medical condition and managing the stressors of their
• situation.
• Identifies the psychosocial barriers to attaining wellness and develops a plan to address those barriers.
• Evaluates psychiatric, behavioral, abuse, chronic disease, chemical dependency and end of life situations.

Utilization Management
• Helps patients avoid unnecessary hospital bills by providing insurance companies with medical information to assure proper reimbursement.
• Researches insurance denials of payment.
• Helps patients understand their bill when medical questions arise.
